GovBenefits.gov

This is a nice program that links people with government assistance programs on the state and federal level.  It's not only for people who have disabilities, but for anyone that thinks they may qualify for some extra help.  There are 66 questions for you to answer to determine what services you qualify for.  After you answer all the questions (most are multiple choice), you will get your results.  You don't have to give any personal information, not even your email address.  If you don't have time to take the online screening tool, you can also just browse the programs.  It gives information on many state and federal programs, and information on how to apply.

You will find links on this page for a vast array of helpful information.  There is an online application for social security benefits.  You will also find links to local offices, retirement benefits, and the ticket to work program.  You can change your address with social security here.  You can also apply and get a replacement social security card.
